ATHLETIC PARTICIPATION FORMS

Welcome to Benedictine College Athletics!

On Monday, June 2nd, 2025, you will receive an email containing your onboarding credentials for Healthy Roster. Healthy Roster is a website that will be your patient portal throughout your Benedictine College athletic career. It will also serve as your appointment scheduler with the Benedictine College Athletic Training Staff throughout the school year. Healthy Roster does have a mobile application, per user preference.

Between June 2nd and July 31st, 2025, please log into your Healthy Roster account and create your profile. Then, complete all incomplete forms.

There are 2 medical history and physical packets for Benedictine College student-athletes: one for new and transfer student-athletes and one for returning student-athletes. The forms are found at the links below.  

Physicals will only be accepted on our Benedictine College forms!

Download and print your physical packet from the link below. Complete it to the best of your knowledge and have the healthcare provider review and sign your physical form.

Please note: Physicals must be completed by a physician, physician’s assistant, or advanced practice nurse. The credentials of these healthcare providers are MD, DO, PA-C, or APRN. We cannot accept physicals completed by any healthcare provider other than our own. Make sure the provider completing your physical lists one of these credentials on your physical packet, so it does not get rejected by the Benedictine College Sports Medicine Staff.

Please note: All questions must have answers within the packet. If a question does not apply to you, please write ‘N/A’.

Benedictine College Sports Medicine Staff will review health history information through Healthy Roster in the order in which it is received. If you are missing a form, if a form is completed incorrectly, or if a form is missing one answer, the Sports Medicine staff will reject the form, and you will need to resubmit it for review.
